Senior Family Care Navigator

The Family Caregiver Support Center provides a full range of clinical services to include assessment of family caregivers to older adults with multiple medical impairments or cognitive impairments, including case management, short-term treatment, referral, crisis intervention, and psycho-educational training. Our program is rapidly growing, and we need superstars to join our current dynamic team. We are seeking a Senior Family Care Navigator to join our team!

This is a full-time position that will require substantial commitment and flexibility. Will require strong organizational skills and attention to detail. A valid state driver’s license and reliable transportation are required. Travel will be required for on-campus meetings. The work schedule varies from 8:30 am to 6 pm, Monday through Friday, with some weekend work as needed. This position will be hybrid.

Roles and responsibilities to include:

Supervises Intake and Resource staff and student workers.

Coach staff through challenging customer service issues.

Interact with callers and handling client queries and complaints in a timely manner

Consulting with service providers and community agencies on case management issues.

Assisting in development of a community-based referral network.

Organizing, conducting reviews of community resources, social service agencies, and other appropriate referral sources.

Interviewing clients and others to obtain information to assess client medical and/or mental health needs and recommend treatment and/or service modalities.

Assisting in developing and coordinating implementation of department policies and procedures.

Planning and conducting client file quality assurance reviews and recommending changes as appropriate.

Presenting at educational workshops and psycho-educational trainings to professionals and caregivers.

Maintaining monthly units of service logs to document services provided to meet each grant’s goals.

Communicating program priorities and performance standards and assessing operations using these criteria.

Maintaining information system standards and other systems as needed.

Preferred Qualifications:

Bilingual skills

Supervisory skills

Experience working with family caregivers

Experience developing and conducting professional presentations

Experience working with virtual programs, including Zoom and Teams

This is an exempt position with work expected to be standard hours of operations, with potential for weekend work. Some travel will be required to conduct presentations or participation in community events. This position is funded by a research contract/grant and may be funded by a subsequent contract/grant. Accordingly, your position is contingent upon receipt of the necessary funding from this or any subsequent contract/grant to continue employment.

The annual salary range for this position is $69,000-$74,000. When extending an offer of employment, the University of Southern California considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate’s work experience, education/training, key skills, internal peer equity, federal, state, and local laws, contractual stipulations, grant funding, as well as external market and organizational considerations.
