The WIC Program serves pregnant people post-partum birth parents, infants and children up to their 5th birthday. The program offers preventative nutrition education, breastfeeding education and support and benefits in the form a prescribed list of healthy foods each month. Senior Community Workers in the WIC Program will be responsible for managing the Farmer's Market Nutrition program; acting as a liaison of the local WIC authorized vendors which involves quarterly in-person visits and vendor education; helping with training the WIC Nutrition Assistants (WNAs) and other all-staff training; supporting various quality assurance activities; and creating and managing the standard outreach presentations of the WIC program. he incumbent will be required to travel to various locations to conduct trainings, attend meetings, and carry out in-person store visits and surveys as required by the state. Currently, there is one regular, full-time vacancy at the WIC program. The ideal candidate for the Senior Community Worker of the San Mateo County WIC Program will possess the following: + 2 years of experience in the WIC Program. + Experience in working with infants, children, pregnant women, and parenting women. + The ability to work independently as well as within a multi-disciplinary team. + The ability to problem-solve, make independent judgments, and work autonomously. + Have strong, technical skills in the ability to work with multiple platforms including Canva, WIC WISE, Microsoft programs, etc. + Have the ability to learn various QA tools including BI. + Communicate effectively both orally and in writing including in a large group setting. + Possession of a valid driver license. + Be flexible to adapting to a changing environment. + Ability to read, write and speak in Spanish. Examples Of Duties Duties may include, but are not limited to, the following: + Assess and monitor client needs and develop individual care/recovery/support plan based on the needs of the client. + Prepare and/or coordinate the preparation of informational and outreach materials, basic correspondence and reports, and other documents regarding program activities. + Assist staff in developing strategy for conducting and maintaining effective relationships with the community; interpret community needs and cultural patterns to professional staff members. + Performs crisis intervention by arranging for special services to assist clients in addressing serious personal, financial, employment, and/or family issues; performs crisis intervention. + Establish and maintain collaborative relationships with community resources. + Conduct home and field visits to assess and monitor client needs and ensure adherence to individual care plans. + Provide information and refer clients to appropriate social services agencies; research community resources for clients; work closely with other County staff to coordinate various functions of outreach and referrals. + Provide educational services to the clientele served; schedule, arrange, and assist in the facilitation of educational programs and activities to support client health/wellness, recovery, self-sufficiency, or employment goals. + Perform outreach to provide education and information to individuals who may be at risk and unaware of services and/or prevention methods; inform clients and community members of programs and services and promote participation of groups and individuals in program activities. + Conduct outreach activities to community organizations to provide information about the program and resources and to secure health, welfare, educational, and other social service supportive programs on behalf of clients; promote participation of groups, businesses, and individuals in program activities; and coordinate the delivery of support services to clients. + Identify home, family, or community conditions needing improvement and develop, recommend, and implement measures to correct these conditions. + Assist in the follow up and monitoring of cases for assigned client groups; work with client and case management team to develop and maintain individual development plans. + Monitor action plans and periodically make contact with clients to assess progress and identify support needed to aid clients in pursuing interim goals as planned. + Advocate with external agencies, employers, and other organizations, to help ensure that the client is represented and informed of processes and procedures, to resolve issues and disputes with third parties, as well as engage in communications on behalf of the client. + Assist clients in a wide variety of daily living activities, such as completing applications and forms, providing transportation, budgeting, and navigating support service systems. + Represent the division or section on internal and community committees and task forces as assigned representing the needs of needs of the community or of individual clients; attends and participates in community events. + Act as an interpreter in contacts involving non-English speaking residents. + Attend staff and in-service meetings. + Develop immediate solutions to emergency problems and expedite delivery of needed services. + Perform special assignments, projects, or other related duties as required. Option 1 - Health Education Duties of the positions in this option may also include: + Assist in planning, organizing, and conducting health education programs. + Observe and recommend health education needs and resources. + Evaluate the effectiveness of educational methods, techniques, and materials and recommends modifications. + Work with community groups including parents, schools, and health organizations to create health education programs. + Assist in the planning and preparation of educational materials such as flyers, pamphlets, exhibits, and displays. Education and Experience: Any combination of education and experience that would likely provide the required knowledge and skills is qualifying. A typical way of gaining the knowledge and skills is two years of experience providing community services of a remedial and educational nature which includes experience counseling and conducting outreach to the client population served. Licensure/Certification: Some positions may require possession of a class C California driver license or equivalent. Physical Demands: Must possess mobility to work in the field as well as an office environment; to operate a motor vehicle for home visits, jail visits, to attend meetings, and visit various County sites; to inspect sites including client homes; vision and smell to assess environmental situations, identify conditions, and to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone. The job involves fieldwork requiring frequent walking on uneven terrain and climbing and descending structures to access client homes and interact with members of the community. Finger dexterity is needed to access, enter, and retrieve data using a computer keyboard or calculator and to operate above-mentioned tools and equipment. Positions in this classification bend, stoop, kneel, reach, and climb to perform work. Employees must possess the ability to lift and carry materials, objects, and children weighing up to 60 pounds, with the use of proper equipment. Environmental Elements: Employees frequently work outdoors and are exposed to varying weather conditions, loud or excessive noise levels, unpleasant odors, dust, fumes, allergens, vermin, parasites, or insects, toxic substances; infectious agents, and cold and/or hot temperatures. Incumbents may be exposed to blood and body fluids performing duties. Protective clothing such as gloves, masks, or gown may be necessary when performing client assessments or when supporting client job training.
