Job Summary

Responsible for ensuring the safe, successful, and timely coordination of patients across Spectrum Health. Supports safe bed assignment and patient flow based on the clinical needs of patients utilizing appropriate hospital resources. Coordinates and evaluates patient flow processes relating to taking, accepting, and triaging of calls as well as resolves operational issues surrounding patient placement and throughput by collaborating with clinical and operational teams.

Essential Functions

Coordinate all aspects of patient transfers between healthcare facilities (hospitals, long-term care, medical offices, etc.), departments and/or nursing units. Using complex acuity protocols for time sensitive medical events, obtains pertinent information to identify appropriate level of care needed for patient placement and safe patient transport. Serve as a communication link between physicians/ED staff/inpatient unit staff and provider office staff as necessary to assure appropriate access and resources.Plan, prioritize and direct patient transfer and placement requests by evaluating pending discharges, transfers and bed availability and collaborating with operational leadership to ensure timely transfer. Communicates with medical staff, clinical and operational teams to facilitate appropriate placement of inpatients and efficient patient throughput. Escalates barriers through established chain of command.Utilizes knowledge and understanding of acuity of disease processes, in addition to knowledge of emergency critical care of adult, adolescent, pediatric and geriatric patients to triage appropriately and communicates clinical updates to clinical staff.Maintain continuity and accuracy of documentation. Maintain service level commitments by handling calls in a timely manner. Adheres to appropriate standards of practice, policies and procedures.Actively participates in process improvement initiatives to improve patient safety and outcomes. Uses safety behaviors to maintain a safe and high reliability workplace.Acts as a resource to support the Transfer Center department functions and assists with training and mentoring new staff.

Qualifications

Required Associate's Degree or equivalent Nursing education requirements for charge nurse duties are the same as specified in the job description for the staff nurse role within the department. Refer to parent job description for requirements.Preferred Bachelor's Degree Nursing3 years of relevant experience Nursing experience within a hospital setting including but not limited to inpatient adult and pediatrics, acute care or critical care, emergency/urgent care.
