As an integral part of our clinical team, the RN will provide high-quality nursing care in a primary care setting, working collaboratively with providers and support staff to ensure the best outcomes for our patients and their families. Key Responsibilities: + Provide direct patient care including triage, medication administration, immunizations, and health screenings. + Assist with routine examinations, minor procedures, and chronic disease management. + Collaborate with physicians and other clinical staff to develop and implement patient care plans. + Educate patients and families on diagnoses, treatments, medications, and healthy lifestyle choices. + Maintain accurate and up-to-date documentation in the electronic medical record (EMR). + Adhere to all safety, infection control, and nursing policies and procedures. + Participate in quality improvement initiatives and departmental meetings. + Support care coordination efforts and follow-up for patients with complex healthcare needs.
