Our interdisciplinary Home Hospital team delivers care to patients who meet national acute inpatient guidelines. Within this team, Nurse Navigators play a vital role by identifying and screening eligible patients in the Emergency Department and Hospital Inpatient units, coordinating admissions alongside the Home Hospital Provider, Case Manager, and Administrative Support Team.
As the first point of contact and the face of the Home Hospital program, the Nurse Navigator serves as a trusted liaison between the Emergency Department and Hospital Inpatient Care teams, patients, families, and Case Managers. This role requires a deep understanding of the program and the ability to clearly communicate its benefits, answering questions and easing concerns to ensure a smooth and confident transition.
Present and proactive in the Emergency Department and Inpatient Units, the Nurse Navigator actively participates in rounds and huddles and independently identifies candidates throughout each shift. This critical role drives the growth and success of the Home Hospital program by ensuring timely, patient-centered identification and engagement.

Job Summary
Key Responsibilities· Attends daily Home Hospital team huddles to review patient status.
· Collects relevant clinical information to identify eligible patients within the catchment area.
· Collaborates with the ED and Hospital Care teams to support patient identification and transfer to MGB Home Hospital.
· Coordinates with the Home Hospital Admitting Provider and Case Manager for patient eligibility and transfer logistics.
· Obtains patient consent for transfer to Home Hospital.
· Communicates with Administrative Coordinators and Managers regarding transfer processes.
· Maintains real-time awareness of Home Hospital capacity and capabilities.
· Supports patient flow and placement to meet Home Hospital census goals.
· Applies critical thinking and sound judgment aligned with organizational objectives.
· Acts as a role model, coach, and facilitator promoting teamwork, respect, and collaboration.
· Demonstrates effective communication that fosters patient-centered care, relationship-building, and professionalism.
Qualifications
36 hour position- 12 hour shifts

Requirements
· Current, active RN licensure in the state of Massachusetts
· 3+ years of experience working in an emergency department, home hospital, and/or inpatient setting
