Position Overview: We are seeking a dedicated and compassionate Care Manager to join our Primary Care Health Home (PCHH) program. This state-funded initiative aims to support patients with Medicaid and specific health conditions, helping them avoid hospital and emergency department visits through close case management. The health conditions include diabetes (DM), behavioral health issues, obesity, asthma, and high blood pressure.
Key Responsibilities:
Patient Support: Manage a panel of up to 250 patients, primarily at the Troy and Warrenton GlennonCare primary care clinics.Communication: Provide support through phone calls, refill management, specialist appointment scheduling, Epic messages, and face-to-face interactions during clinic appointments.Case Management: Develop and implement personalized care plans to address patients' health needs and ensure they receive appropriate care.Coordination: Collaborate with healthcare providers, specialists, and community resources to ensure comprehensive care for patients.Documentation: Maintain accurate and up-to-date patient records in the Epic system.Qualifications:
Education: Bachelor's degree in NursingExperience: Previous experience in case management, particularly with Medicaid patients and the specified health conditions, is preferred.Skills: Strong communication, organizational, and problem-solving skills. Proficiency in using electronic health records (EHR) systems, especially Epic, is a plus.Attributes: Compassionate, patient-focused, and able to work independently and as part of a team.Work Environment:

Provides services as a patient navigator throughout the continuum of care.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts.Compiles data, tracks outcomes, and makes recommendations for process improvement.Provides referrals/support for follow-up care for patients who are uninsured or underinsured.Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.Establishes working relationships teaching protocols for patient education, family counseling and general public information.Works in a constant state of alertness and safe manner.Performs other duties as assigned.EDUCATION
Graduate of accredited school of nursing or education equivalency for licensingEXPERIENCE
