CoverageArea: Walker, Trinity, Polk, San Jacinto-may be asked to help other territorieswithin 60 miles radius from address

Productivityexpectation: 4-5 visits daily

Musthave: Hospice experience, HomeCare HomeBase experience, LIC in hand
