This role will support lower Cuyahoga county and Summit county.
As an RN, you are responsible for delivering high quality support and medical services in a comfortable home setting. In this role, you will provide assessments, various treatments and education to help avoid readmissions to the acute care setting. After seeing patients, you will complete documentation, make calls, attend meetings, coordinate additional care and arrange visit schedules. This position offers an excellent way to not only enhance the healthcare experience but also further develop your skills as a compassionate and dedicated nurse.
A caregiver in this position works a standard schedule of 8:00am to 5:00pm with a weekly on-call rotation, serving as the primary contact from 5:00pm to 8:00am or secondary from 5:00pm to 9:00pm via pager. This role also includes participation in a weekend rotation every fourth weekend and coverage for 1 summer and 1 winter holiday per year.
A caregiver who excels in this role will:
Perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames.Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.Perform medication reconciliation at each visit.Assess patients for safety risks, abuse and neglect.Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness/timeliness of care and interventions.Report abnormal findings and/or changes in patient condition to the physician and update care coordinators with those findings.Communicate any patient fall out/missed visits to the care coordinator immediately.Participate in and support quality improvement projects and department committees as assignedMinimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of professional nursing. Current state licensure as a Registered Nurse (RN). Basic Life Support (BLS) certification through the American Heart Association (AHA)One year of clinical experience as an RN in an acute med/surg setting. Valid Ohio driver’s license and proof of automobile insurance coverage with $100,000/$300,000 coverage. Minimum one year current clinical experience as a registered nurse.Minimum one year experience in an acute medical-surgical or home care setting.Preferred qualifications for the ideal future caregiver include:

Physical Requirements:
