RN Home Health Case Manager Hospice

Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Cleveland Clinic's hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. This team works together to support patients wherever they call home. As an RN, you will provide supervision, assessment, planning, and hands-on care to hospice patients, following agency policies and the care plan set by the team. This opportunity allows you to demonstrate compassion, empathy, and a nurturing attitude while building meaningful relationships with patients, families, and fellow caregivers, all while gaining growth and development opportunities for your dedication. **A caregiver in this position works** **8:00 a.m. - 5:00 p.m. with weekend rotation every 10 weeks and one rotating summer and winter holiday per year required.** A caregiver who excels in this role will: + Provide supervision, assessment, planning and direct patient care to hospice patients. + Perform accurate and complete admissions assessments and re-assessments. + Assess patients for safety risks, abuse and neglect. + Assess, document and manage patient's pain. + Prioritize and document patient problems. + Administer medications and treatments safely and accurately with a focus on patient comfort. + Coordinate care for managing a caseload of patients. + Document changes in patient condition and effectiveness and timeliness of care/interventions. + Evaluate, update and revise plans of care. + Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges. + Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs. Minimum qualifications for the ideal future caregiver include: + Graduate from an accredited school of nursing. + Current licensure as a Registered Nurse (RN) + Basic Life Support (BLS) certification from the American Heart Association (AHA). + One year of current clinical experience as an RN. + One year of experience in an acute medical-surgical or critical care setting. + Valid state driver's license. + Proof of automobile insurance with $100,000/$300,000 coverage. Preferred qualifications for the ideal future caregiver include: + Bachelor of Science in Nursing (BSN). + Two years of critical care, and/or med/surg experience. + Hospice and/or oncology experience.
