**Position Profile**
The RN Dementia Educator provides expert dementia education and support to patients/families/caregivers involved in the Supportive Care for Dementia Program, with the goals of supporting comfort and quality of life for both persons with dementia and their caregivers.
**Responsibilities**
+ Provides expert palliative dementia care education and support in consultation with the Supportive Care for Dementia Director/Medical Director.
+ Provides effective patient/family/caregiver teaching.
+ Provides effective dementia and delirium assessment and management.
+ Promote patient safety.
+ Promotes Hospice of the Valley’s Dementia Programs in the Community
+ Develops and maintains therapeutic relationships.
+ Provides comprehensive psychosocial assessments.
+ Provides emotional support.
+ Creates timely and accurate documentation.
+ Utilizes resources effectively and efficiently.
+ Maintain and enhances professional skills.
+ Adheres to high standards of personal and professional conduct.
**Minimum Qualifications**
+ Graduate of accredited school of nursing.
+ Current Arizona RN licensure in good standing.
+ Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
+ Knowledge base of dementia including causes, symptoms, and management of dementia and family support strategies.
+ Demonstrated understanding of issues of terminal illness; death and dying; loss and grief.
+ Awareness and sensitivity to cultural diversity.
+ Knowledge of self care, personal boundaries; abilities, limits and inner resources.
+ Strong clinical assessment skills.
+ Excellent communication and interpersonal skills.
+ Solution driven, creative and resourceful problem-solving skills.
+ Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
+ Proficient computer skills.
**Preferred Qualifications**
+ Experience working in a setting which includes persons with dementia is preferred.
