The Registered Nurse Coordinator (RN) in the clinic setting provides high-quality patient care through routine nursing duties, clinical assessments, and collaboration with healthcare providers. This role involves triaging patient concerns via telephone and/or in-person interactions, documenting symptoms, medical history, and medications in the electronic medical record while identifying potential concerns and initiating appropriate care plans. The RN demonstrates professionalism, ethical clinical behavior, and respect for patient rights while maintaining confidentiality and adhering to HIPAA regulations. Strong critical thinking, communication, and time management skills are essential for success in this role. Experience with cardiac, thoracic or vascular patients preferred.
This is not a leadership role, this position does not have direct reports.
Essential Functions
* Assesses patients during the intake process, documenting symptoms, medical history, and medications in the electronic medical record while identifying potential concerns and initiating appropriate care plans.
* Triages patient concerns via telephone or in-person interactions, providing recommendations or escalating concerns to the appropriate care team member.
* Communicates effectively with patients, families, and healthcare team members to ensure seamless care delivery and a supportive patient experience.
* Creates and delivers patient education on disease processes, treatment plans, and medication management to promote understanding and adherence.
* Coordinates follow-up care by responding to patient portal messages, returning phone calls, and addressing test results such as labs, imaging, or medication refills.
* Collaborates with interdisciplinary healthcare teams to develop and implement care plans, advocating for patient health and well-being.
* Performs other duties as assigned.

**Responsibilities**
Coordinate nursing program activities and resources within clinical specialty or department.
+ Develop and oversee program, making recommendations and acting as a resource for all nursing staff related to patient care and clinical standards, policies and procedures. Develop and document care plans and therapy programs.
+ Oversee and ensure plan of care therapy program is implemented as indicated. Ensure necessary resources and/or equipment is obtained as ordered.
+ May supervise and direct nursing staff.
+ Coordinate, assemble, and maintain documentation as required.
**Requirements**
Licensure / Certification / Registration:
+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
+ Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
+ Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
Education:
+ Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
