**Description:**
**POSITION PURPOSE:**
The Advanced Practice Provider is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all spine patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.
**REQUIRED EDUCATION, EXPERIENCE AND** **CERTIFICATION/LICENSURE**
Education:
+ Graduate of an accredited school of Physician Assistant/MSN program or other Masters of Science degree in a health related field including six (6) to twelve (12) months of clinical internship
+ Graduate of an accredited Nurse Practitioner program with a minimum of a Masters of Science in Nursing, including six (6) to twelve (12) months of clinical internship.
Credentials/ Licensure
+ Must hold minimum licensure and in "good standing" in one of the following in the State of Michigan: Physician Assistant or Nurse Practitioner.
+ If Physician Assistant, must hold certification from the National Commission of Certification.
+ Current Advanced Cardiac Life Support (ACLS) certification.
+ Must be credentialed for clinical privileges through the SJMO AHP Credentials Committee and the SJMO Board of Trustees.
Experience:
+ Minimum of one year of experience in spine in an acute care setting.
+ One year of experience in medical, surgical acute care.
+ Case management experience or patient care leadership experience preferred
+ Five years of experience in subspecialty.
+ Experience with current spine care guidelines
**ESSENTIAL FUNCTIONS AND RESPONSIBILITIES**
**_Patient-Care_**
Understands and develops goals, treatment plan and provider options in concordance with evidence based standards Works directly with physicians to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care, on an as needed basis to assist in patient throughput Identifies patients at high risk for treatment failure or interruptions and enlists additional resources. Serves effectively as a patient and family advocate. Promotes team conferences for patients. Supports patient during difficult decision-making periods. Assists in coordination of end of life care patient and family. Performs post-operative phone call / telemedicine visit to spine patients to prevent readmissions Educates and coordinates patient education for Pre-Operative Spine classes. Educates and coordinates staff education on spine care in conjunction with service line CNL
**_Administrative_**
Identifies quality and process issues and uses data and evidence to drive decisions at point of care that improve and optimize care delivery for patients, clinical outcomes, and patient and nurse satisfaction. Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes to measure program effectiveness. Assists the Director of Neuroscience with Joint Commission (or other regulatory body) data collection and site visits for initial or re-certification processes for Spine. Prepares, maintains and improves upon requirements to obtain and sustain Spine Center of Excellence certification Maintains registry of all spine patients and coordinates collection of data with physicians and their offices. Establishes and promotes collaborative relationships with physicians and other members of the health care team. Develops and promotes collegial relationships with customers. Models personal and professional growth by continuing education and obtaining certification in an area of specialization Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, associates, and visitors to St. Joseph Mercy Health System. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Behaves in accordance with the Mission, Vision and Values of SJMHS. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

**Minimum Education, Licensure / Certification and Experience Required.**
**A. Education**
**•** Master of Science in Nursing.
**•** Graduate Degree in Nursing Education or Certificate for Nursing Education preferred.
**B. Licensure / Certification**
**•** Current RN licensure in the State of Michigan.
**•** Basic Life Support (BLS) certification. Advanced Cardiac Life Support (ACLS) certification preferred.
**•** Certification by a nationally recognized nursing organization is preferred.
**•** Membership in a professional nursing organization is preferred.
**Minimum Education, Licensure / Certification and Experience Required.**

using critical thinking skills, and advanced knowledge with emphasis on patient safety and evidence-based

Ability to serve as a teacher, mentor, and coach to patients, students, new nurses, peers, other colleagues, and
staff, to advance the profession of nursing and optimize patient outcomes.

**•** Clinical expertise in a specialty or sub-specialty of nursing practice.
Basic Life Support (BLS) instructor and/or faculty status required within 6 months of hire. Advanced Life
Support instructor and/or faculty status required for areas of clinical practice that staff certification is
required within 6 mon-1year of hire.

**D. Experience**
Three years in clinical nursing practice in Adult General Medical/Surgical and/or Adult Critical
Care/Emergency and/or Maternal Child Health and/or Surgical/Peri-operative area or other specialty nursing
practice experience required.
**•**
Previous educational experience as a Clinical Nurse Specialist, preceptor, unit educator, central nursing
education role or academic clinical instructor required.
