The main responsibility for this position is to reconcile the medication list that a patient is taking from the referral and discharge summary. The RN will review the medication list that is created by a Physical Therapist after an admission is performed. The nurse will contact the patient /caregiver and the provider/pharmacy with any discrepancies or findings. The nurse will review risks, and adverse reactions of medications and educate patient/caregiver over phone.

Requirements:

NYS RN LicenseValid NYS Driver's LicenseExperience in geriatric diagnosis and medicationsOne year of pervious experience in the health care fieldMust be knowledgeable in medications and be able to teach over phoneMust be able to communicate with the Physical TherapistMust be able to multitask, including tracking and following through on medication changesMust have good attention to details and processes

Preferred:

EPIC background a plusKnowledge of regulations for a certified home health agency a plus

Pay Range:

$33.10 - $47.15

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
