Links the progression toward discharge with the clinical improvement of the patient. Coordinates clinical care to include discharge planning, medical necessity, length of stay management. Collaborates with providers, nursing, ancillary staff, patients and families to achieve optimal clinical outcomes. Executes clear, complete and concise documentation of all interventions and assessments.
