As a RN Case Manager at MercyOne, you work with the interdisciplinary team for patients on a select unit from admission to discharge. This care will be determined by individualized plans of care, care guidelines, best practice data and critical paths.

General Requirements:

Mobilizes resources and manages the systems to achieve outcome-based careCoordinate physician, patient and payer interactions to ensure appropriate level of care.Respond to concurrent and retrospective data analysis identifying trends in patient care for that unit’s patient population.

Education:

Active IA RN licenseBSN preferredMinimum of 4 years of clinical experience.
