The RN Case Manager performs a wide variety of duties and responsibilities in a manner that places emphasis on developing and coordinating individualized patient care services.
As part of the essential functions of this role, the RN Case Manager:
+ Provides nursing support and care management services to patients presenting for injectable medications, including coordination of medication delivery, ensuring that required lab work is complete, and scheduling and arranging follow-up appointments
+ Identifies and/or develops injection education and supportive interventions to increase patients' skills and confidence in managing their health, including regular assessment of progress and setbacks, goal setting, and relapse prevention support
+ Educates patients, including medication administration, storage, safety, responsibilities, expectations, and patient treatment agreements
+ Assists with patient medication refills and complying with NYS PMP (I-STOP) regulations
+ Acts as a liaison with pharmacy; completes and assists in prior authorizations
+ Resolves patient problems and needs by utilizing multidisciplinary team strategies
+ Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations
+ Acts as the liaison to Erie County Department of Health, New York State Department of Health and other public health entities relating to patient treatment, follow-up of care, and surveillance; maintains records, prepares reports, and composes correspondence relative to the work
+ Instructs and informs nurses regarding nursing practice in accordance with relevant policies and guidelines; works directly with nursing staff to provide guidance and education, including facilitation of learning by using assessment and evaluation strategies
+ Helps to ensure that all nursing staff receive required and recommended trainings necessary to provide testing, referral, administration, and/or referral to treatment, related documentation, and any necessary follow-up or required reporting/surveillance
+ Acts as an advocate for the patient in all activities, including nursing assessments, care coordination, care plan development, and communication
+ Educates patients in chronic disease management based on evidence-based protocols
+ Develops and implements care plans and provides intense education for medically complex patients, including developing interventions to address gaps in care and monitoring patients' response to interventions
+ Completes transition of care (TOC) follow-up and regulatory requirements.
Qualified Candidate will have a current license as an Registered Nurse in New York State; Bachelor of Science in Nursing (BSN) OR a documented plan to complete a BSN within NYSED's RN BSN requirement timeframe OR Associate degree with degree/license dates meeting NYSED’s exemption requirements; Basic Life (BLS) Certification required; at least (3) year of experience working in an medical office setting, in an HIV/AIDS or other infectious disease practice preferred; candidate must have familiarity with computers and MS Office software (Outlook, Word), with EMR software (Medent) preferred; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential.

Required certification: Basic Cardiac Life Support (BCLS)
