Baystate Health is hiring a Hospital Case Manager (RN) to help patients navigate their care with compassion, efficiency, and clinical expertise. If you’re an experienced nurse ready to take the next step in your career, this is your opportunity to lead care coordination and make a difference.

Position Highlights:

Collaborate with patients, families, doctors, and care teams to develop and manage patient-centered plans

Assess needs, coordinate discharge planning, and ensure safe transitions of care

Use clinical criteria to review admissions and ongoing care for medical necessity

Communicate with insurance companies to support timely, appropriate reimbursement

Participate in daily unit rounds to guide care progression

Requirements:

Registered Nurse (RN) with an active license

Bachelor’s Degree preferred (Associate’s accepted with approval)

3–5 years of recent acute care (med-surg) nursing experience

1–3 years of hospital case management experience (preferred)

Strong communication, critical thinking, and teamwork skills
