RN Care Manager
Job Title: RN Care Manager | Seattle, WA

Job Description

This role in care management focuses on transitions of care, including coordination and medication reconciliation, aiming to reduce emergency department visits and readmissions. The position involves handling all inpatient discharges, except labor and delivery and oncology follow-up, and addressing all emergency department visits, with particular emphasis on patients with chronic conditions.

ResponsibilitiesCoordinate transitions of care and ensure smooth patient discharge processes.Conduct medication reconciliation to promote patient safety and reduce readmissions.Focus on reducing emergency department visits and readmissions.Manage inpatient discharges, excluding labor and delivery and oncology follow-up.Address emergency department visits, especially for patients with chronic conditions.Essential SkillsValid RN license in Washington State.1+ years of experience in Acute Hospital, Long Term Acute Care, Rehab, or Skilled Nursing settings.Proficiency in case management and care coordination.Experience with EPIC software.Utilization review and appeals handling capabilities.Discharge planning and population health management.Additional Skills & QualificationsBasic Life Support (BLS) certification.Work Environment

The position is based in an outpatient clinic with a hybrid schedule: 2 days per week on-site at the Beacon Hill clinic and 3 days per week working from home. The clinic will provide all necessary equipment for remote work.

Pay and Benefits

The pay range for this position is $40.00 - $60.00/hr.
