Job Description
+ Deliver telephonic case management services, coordinating care across PCPs, specialists, and support staff to address clients’ medical and socioeconomic needs.
+ Educate patients and providers, facilitate access to community-based services, and monitor referrals to ensure alignment with individualized care plans.
+ Document and manage clinical data including assessments, authorizations, and care notes in medical management systems, while identifying and reporting quality or risk concerns.
+ Collaborate in interdisciplinary care rounds and committees, contributing to special projects and applying advanced knowledge of complex care management independently.
Qualifications
+ Associates/Bachelor's Degree in Nursing
+ Active RN License
+ 2+ years of clinical nursing experience in home health, hospice, managed care or case management
+ Experience working with people with disabilities and vulnerable populations who have chronic or complex conditions
+ Bilingual in Spanish and English is highly preferred
+ Prior Home Health experience is highly preferred
Work Environment:
The work environment is fully remote and involves interaction with members. Familiarity with case management processes and managed care organizations is essential.
Pay and Benefits
The pay range for this position is $35.00 - $45.00/hr.
