Retail Pharmacist - Specialty Pharmacy - Full Time - Days
REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES Reports to Pharmacy Manager with no direct reports. POSITION SUMMARY Provides pharmacy services, to include drug distribution and dispensing, drug information, drug therapy monitoring, patient education, and controlled substance regulation. Responsible for providing direct supervision of supportive personnel within the Department of Pharmacy. Actively participates in the creation of an environment that fosters patient, family, physician, and staff satisfaction. Ensures decisions made are based on the Mission and Values of ProMedica. ACCOUNTABILITIES *All duties listed below are essential unless noted otherwise* Reviews prescriptions issued by physician, or other authorized prescriber, to ensure accuracy and appropriateness. Compounds medications using standard formulas and proper aseptic technique. Monitors all new medication orders against the patient’s medication profile for contraindications, drug–drug interactions, drug–food interactions, drug–laboratory test interactions, allergies, and appropriateness of drug, dose route, frequency, and therapeutic duplications. Resolves identified problems with the physician or nurse; and documents these activities as appropriate. Answers all drug information questions accurately using drug information resources available in the department and documents these activities as appropriate. Reads, extracts, and interprets information in patient charts accurately. Documents all clinical activities and interventions accurately and completely. Obtains required continuing education for state licensure. Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner. Conducts patient education/counseling to inpatients and/or outpatients as requested. Collaborates with other team members in meeting departmental goals. Establishes and maintains mechanisms for communication and constructive problem solving with staff. Utilizes a team approach in decision-making and problem solving. Builds trust and mutual respect. Keeps director informed of any unexpected developments that may affect patient care or the budget of the department. Supervises and directs pharmacy support personnel; verifies the daily activities of pharmacy technicians; and participates in the performance management process as requested. Supervises and instructs students. Adheres to state and federal laws. Performs all duties and responsibilities in an ethical and responsible manner. Supports the department in providing a vision for its direction and embraces the change process to achieve the vision. Effectively makes decisions to achieve desired outcomes. Coaches and facilitates the growth and development of employees within the department. Directs daily operations to achieve goals for the department. Strives for continuous improvement by focusing on key department systems. Develops an environment for success through team participation. Other duties as assigned. Specialty Pharmacy Only Completing and submitting prior authorization forms with great attention to detail and accuracy. Assisting prescribers with denial/appeal of reject claims. Gather proper information and completing patient assistance applications. Works proficiently within pharmacy and specialty workflow software systems. Perform “New Patient Start” calls which include, but are not limited to medication reconciliation, disease state evaluation, concomitant disease state evaluation, assess clinical appropriateness, and education/training. Assess appropriateness of subsequent refills, address patient issues/concerns, monitor adherence, and create care plan when appropriate. Shipping completed orders to patients and prescribers. Participates in departmental performance improvement activities. Participates in e-learning activities. Maintains ongoing knowledge, understanding, and continuing education of related disease states and medications pertinent to specialty pharmacy.
