**Job Description**
Support patients in maintaining their independence and mobility by delivering restorative therapy services under the direction of licensed therapists. You’ll work 1:1 or in small groups to assist with self-care, movement, and contracture management—always aiming to help residents reach their highest functional potential. You'll also document progress, report changes, and ensure a clean and safe therapy environment.
**Key Responsibilities:**
+ Provide restorative care as directed by PT, OT, or speech therapy
+ Assist with mobility, ADLs, positioning, and cognition/memory programs
+ Monitor and report changes in patient function
+ Support CNAs with functional maintenance programs
+ Maintain cleanliness and organization in therapy spaces
+ Ensure care is age-appropriate and centered on patient safety
+ Participate in quality and compliance initiatives
**Minimum Requirements:**
+ High school diploma or equivalent
+ At least one year of medical experience (rehab or CNA in LTC/hospital setting)
+ Strong communication and documentation skills
+ Team-oriented, reliable, and able to follow directions
+ Organized and proactive in daily responsibilities
**Preferred Qualifications:**
+ Certified Restorative Nurse Assistant or equivalent
+ Background in restorative nursing principles
+ Familiarity with long-term care restorative philosophy
+ Relevant college coursework in patient care
