We work with members throughout Wisconsin to find providers that fit each of their needs. Our goal is to help individuals be in more control of their own supports and to have a say in who is assisting them. This helps them to stay in their own homes as long as possible.
Title: Date Night Provider Needed

Job Posting:We are looking for a compassionate, energetic respite provider to support a bright and spunky little girl. The ideal provider will help her build on her communication skills and encourage her physical abilities through fun, interactive activities—whether at home or out in the community. This is a rewarding opportunity for someone who enjoys forming meaningful connections, has patience and creativity, and wants to make a positive difference in the life of a spirited little girl. Pay: $17.66/hour (after tax rate) Location: Respite to take place in the child's home & community (Eau Claire, WI) Schedule: 10 hours/month available - flexible shifts, weekends preferred - schedule TBD between family & provider Things She Enjoys:Tv/movies/characters: Mickey Mouse & FrozenActivities: Swimming & being outside Reading Walks Jumping on the trampoline Bubbles Going to brothers sporting events  Going on trips Goals:• Improving her overall communication and prevent regression of her physical abilities by engaging with her. Ideal Candidate:• A kind, energetic, and outgoing individual (preferable with special education experience) that wants to spend time with a spunky little girl. • Open to regular communication with the family and adaptable to a changing schedule. Requirements:• Valid driver’s license and reliable transportation • Willing to undergo a caregiver background check upon hire *Must be able to successfully pass a background check before being hired. If hired, Consumer Direct Care Network (CDCN) will not be your legal employer on record. If hired by a Member or their Managing Party, your legal employer on record would be the Member. CDCN’s role is to assist the Member or Managing Party in recruiting a Direct Care Worker. The Member or Managing Party has the sole authority to hire or dismiss any potential Direct Care Worker.
