Description:Position Purpose:

This position will be located in the new state of the art COPD Clinic, and a part of our grand opening of our new comprehensive pulmonary rehab program. The role of the pulmonary rehab respiratory navigator in providing a range of care and therapy interventions for people who have Chronic Obstructive Pulmonary Disease (COPD), Bronchiectasis and other chronic lung conditions. Develop effective, engaging, and motivating educational topics enabling the patient to best self-manage chronic lung disease.What you will do:                          Serve to promote best practice in pulmonary patient care and assimilate various service lines to meet patient needsAbility to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.Provide planned interventions and therapeutic rehabilitation program with adults suffering from COPD, Bronchiectasis and other chronic lung conditions.Provide administrative support for running the pulmonary rehabilitation courses.
