Remote Telehealth LPN/LVNJoin the Home Health Team at Elara Caring as a Telehealth LPN!
Are you a Licensed Practical Nurse (LPN/LVN) who enjoys making a difference—without leaving home? We’re looking for a compassionate PRN Telehealth LPN to join our Remote Patient Monitoring (RPM) team. This is a fully remote, weekend-only position, ideal for LPNs who are detail-oriented, tech-savvy, and committed to patient-centered care.
Schedule: Monday through Friday, 8:00 am CST to 5:00 pm CST (with one weekend per month)
What You'll Do:
Monitor and assess patient vitals remotely using digital devices
Provide nursing support via phone or video
Document all patient interactions and escalate when necessary
Communicate with RN Case Managers and care teams
Deliver education and guidance to patients and families
What We’re Looking For:
Active Compact LPN/LVN licenseMust hold a Compact Nursing License (we serve multiple states) & willing to get licensed in noncompacted States
Minimum 2 years of LPN experience (home health highly preferred)
Must have 1 year of telehealth/virtual nursing experience
Comfortable with telehealth technology
CPR certification (AHA or Red Cross)
Strong communication and assessment skills
Experience with HCHB is a PLUS!
