Remote LPN Care Manager
Job Title: LPN Care ManagerJob Description As an LPN Care Manager, you will be responsible for providing day-to-day care management activities, including member assessments, care plans, and assisting with member needs. You will also support the care management team and maintain communication with supervisors on a daily basis. The role requires decision-making based on internal protocols under the supervision and direction of a medical director. Responsibilities + Conduct day-to-day care management activities including calling members. + Complete member assessments and develop care plans. + Assist with member needs and provide support to the care management team. + Communicate daily with supervisors. + Make decisions based on internal protocols under the supervision of a medical director. Essential Skills + Licensed Practical Nurse (LPN) certification + Registered Nurse experience + Proficiency in computer skills + Case management expertise + Pediatrics experience + Knowledge of Microsoft Suite + Experience in managed care + Acute care experience Additional Skills & Qualifications + Ability to effectively communicate + Demonstrated time management skills and priority setting + Ability to create and interpret care/treatment plans + Proficient in using computers and technology Work Environment This position requires working Monday through Friday from 8 AM to 5 PM, totaling 40 hours per week. The role is remote until at least September 2021, with potential travel next year depending on COVID-19 cases. A strong WiFi connection, work space, and smart technology are essential for this role. Pay and Benefits The pay range for this position is $30.00 - $36.06/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
