**Work location:** Texas Health Dallas 8200 Walnut Hill Lane, Dallas, TX 75231
**Work hours:** Full Time Days: 40 hours/week; 8:30am-5:00pm

**Here’s What You Need**
+ Bachelor's Degree Nursing or Rehab discipline Required
+ 5 Years’ Experience in related clinical care discipline and/or case management Required
+ RN - Registered Nurse Upon Hire Required Or
+ PT - Physical Therapist Upon Hire Required Or
+ OT - Occupational Therapist Upon Hire Required Or
+ SLP - Speech Language Pathology Upon Hire Required And
+ BCLS - Basic Cardiac Life Support Prior to working independently and maintained quarterly Upon Hire Preferred
+ Excellent verbal and written communication skills including the ability to communicate effectively and professionally across disciplines and with a variety of distinct constituents.
+ Proficient computer and Microsoft Office skills.
**What You Will Do**
+ Performs intake assessments for hospital and externally as needed.
+ Reviews patient chart and talks to patient, family, and/or health care providers about admission to the inpatient rehabilitation unit.
+ Documents consultation in medical record.
+ Maintain rehab admissions log capturing information needed to demonstrate CMS appropriate admission compliance.
+ Submit quarterly reports to the Rehab Steering Committee.
+ Responsible for the concurrent chart audit process used for demonstrating medical necessity of the rehab stay.
