Registered Practical Nurse Regular Full Time IBC Behavior Support Ballycliffe

At AgeCare, we are dedicated to creating vibrant communities that celebrate and support aging with dignity, purpose, and joy. We are currently looking for a compassionate and experienced Behavioural Support Coordinator (RPN) to join our care team.

About the Role:

As a Behavioural Support Coordinator, you will provide leadership and direct support to staff, residents, and families in addressing responsive behaviours through a person-centred, evidence-informed approach. This position plays a key role in enhancing the quality of life for residents with behavioural challenges by promoting safety, dignity, and meaningful engagement.

Key Responsibilities:

Lead and support individualized behaviour management strategies using best-practice frameworks like P.I.E.C.E.S.

Collaborate with interdisciplinary teams and families to develop and implement holistic care plans.

Provide coaching and mentorship to front-line staff.

Conduct assessments and regular case reviews to identify risks and adapt care plans.

Participate in quality improvement initiatives and represent Behavioural Support at committee meetings.

Liaise with external partners and ensure compliance with relevant legislation and standards.

What You Bring:

Current registration and good standing with the College of Nurses of Ontario (RPN).

Demonstrated experience and knowledge in dementia care, mental health, and behavioural support.

Exceptional interpersonal, communication, and coaching skills.

Ability to respond to emergency and high-stress situations with professionalism.

Flexibility to adapt to changing resident needs and scheduling requirements.

Requirements:

Certification or specialized training in behavioural support (P.I.E.C.E.S., GPA, etc.) preferred.

Vulnerable Sector Check and Criminal Record Check required.

Physically able to perform medium-demand work (lifting up to 50 lbs occasionally).

Commitment to ongoing learning and participation in staff development opportunities.
