Summary:
The nurse navigator is a registered nurse who functions as the guide who assists patients and their families as they move through the complexities of the healthcare system and is responsible for coordinating patient care activities for select at risk individuals as well as providing infusion services.
Assumes key role in assuring care on a continuum coordinating patient care activities for individuals by referring patients to other health professionals and support services as appropriate (genetic counselor nutritionist psychiatrist social workers and the like). Ensures team is kept appraised of patient�s condition.
Responsibilities:
Provide individualized support education and information to the patient and family helping to make care seamless continuous and comprehensive through the Brown University Health. Patient teaching may include information regarding the disease process expected side effects of treatment and community resources. Facilitate ongoing communication and education with patient and family before during and after treatment process. Provides for appropriate patient and family education and counseling regarding disease and treatment potential complications signs and symptoms requiring immediate attention.
Serve as a liaison between the patient and medical staff and other supportive staff. Serves as the primary contact and communication link between the patient family medical staff and other members of the care team. Support the patient and family through difficult decision-making periods. Assists in identifying and documenting issues related to transitions of care. Provides ongoing feedback to patients on changes related to their care management. Develops collaborative relationships with care team to ensure continuity of care. Rounds on patient as appropriate and monitors their healthcare experience to ensure all expectations are being met. Will be available to patients throughout their care in the system. Assists in the triage of patients’ calls.rticipates in clinic and departmental quality assurance activities such as but not limited to monitoring and evaluation of care validation of nursing and ambulatory standards policies and procedures.
Maintains advanced specialty training or certifications as required. Completes all required competencies. Participates in staff meetings or other meetings as requested. Maintains appropriate licensure. Assumes responsibility for professional growth in nursing practice specialty.
Demonstrates disease specific knowledge skill and coordination to provide care and guidance to patient population. Work across health care disciplines to assist and delegate when appropriate the need to coordinate patient tests procedures appointments and treatments. Assists and collaborates with team in assisting patients in overcoming barriers to care including logistical transportation education financial counseling referral etc. Performs clinical skills and management functions specifically delegated by the clinical manager.
Other information:
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
BASIC KNOWLEDGE:
Graduate of an accredited School of Nursing Bachelor’s Degree in Nursing preferred.
Must have current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact.
Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes human development stages and cultural patterns in each step of the care process.
Professional and analytical skills in evaluating patients in order to plan and provide patient care and monitor the care management process and treatment plan for each patient.
Ability to communicate effectively (public speaking skills) with diverse audiences including professional staff patients community media and other service agencies.
Interpersonal skills to allay anxiety elicit cooperation and provide for education of patients/families and to effectively function as a member of the healthcare team to coordinate and discuss patient care activities.
Advanced skill in assessment of physical and psychosocial health-illness status of individuals history taking and physical assessment.
Possession of nursing certification preferred.
EXPERIENCE:
For this position as with others in our Sickle Cell Center lived or work experience with diverse communities children youth and adults is recommended.Two to three years of hematology nursing experience in inpatient ambulatory or community nursing preferred.
Relevant experience in sickle cell also preferred.
IV competency preferred.
Work Environment and Physical Requirements:
Exposed to ambulatory patient care environment which includes exposure to communicable diseases however adherence to infectioncontrol policies and procedures minimizes chance of illness or injury.
Independent Action:
Performs independently within the departments policies and practices. Refers specific problems to the supervisor when clarification of departmental policies and procedures.
