*The Impact You Can Make* *Team Impact * The Specialist, Stroke Program reports directly to the Coordinator, Stroke Program. The specialist will assist in the monitoring of the care of stroke patients from arrival in the Emergency Department through stabilization and transfer to a higher level of care or discharge home. This position is responsible for monitoring of data for stroke patients, and the stroke program across the continuum. The specialist will participate on the Stroke Steering Committee, with program education plans, and on any ad hoc performance improvement/quality initiatives as needed with the Stroke Program Coordinator** * * *The Glens Falls Hospital Impact* *Mission * Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * *How You Will Fulfill Your Potential* *Responsibilities * * Maintains a comprehensive log of stroke cases presenting to the hospital and ensures a comprehensive review of care against evidence-based standards. * Performs patient follow-up calls to assess patient outcomes, trend program outcomes, and assess patient satisfaction * Responsible for ongoing monitoring of key performance metrics, including process of care measures, clinical outcomes, timeliness measures, and overall quality of care. Deficiencies will be reported to the Stroke Program Coordinator.** *_Education & Experience_* * Registered Nurse with a BSN required * Minimum of 3 years of experience in acute and/or critical care/emergency nursing practice required *_Licenses, Certifications, Registrations_* * Licensed as a Registered Professional Nurse in New York State. *_Skills and Abilities_* * Computer skills, including MS Office Suite * Data analysis and interpretation of statistical reports * Knowledge of adult learning principles * Demonstrated ability to present and teach individuals and groups of patients/families and staff * Strong written and verbal communication skills * Ability to establish and maintain positive interpersonal relationships with patients and professionals of all disciplines *Communities We Serve* Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George.
