If this is your passion, you need to consider joining our team driven to provide quality care and services within a caring Christian culture. **Summary** + The Diabetes Management Educator is responsible for the diabetes education program for inpatients and outpatients and is also responsible for the education of the professional staff related to diabetes. **General Hours of Work** + Monday - Friday Days (occasional evening/weekend shift involvement) **Education and/or Experience** + RN degree from a two-year college/university. Minimum of 1 year nursing required, prior experience in patient teaching or related field preferred. Computer knowledge a plus. Must have academic preparation and/or experiential preparation in program management and in the care of persons with chronic disease. Bi-lingual experience preferred but not required. **Certificate, Licenses, Registrations** + Licensed as a Registered Nurse in the State of Iowa. Must obtain CPR certification within 3 months of hire date. Certified Diabetes Educator with BSN preferred. **Essential Functions** + Assesses department/job category needs, develop curriculum, implements activities, and evaluate programs. + Revise program as needed to ensure a quality product. Collaborate with clinical leaders to assess, plan, implement, and evaluate goals, objectives, and content of clinical education programs. Serves as staff and/or community in-service instructor for educational activities depending on topic and expertise. + Relates curriculum content to current knowledge, practice, and research for a specific job category. + Coordinates experiences with appropriate unit/department to meet employee and organizational goals. + Evaluate the diabetes program content and effectiveness annually internally and with the Diabetes Advisory Board. + Monitor and evaluate the quality of patient care through patient contact, records, and reports, and take corrective measures as necessary to include but not limited to additional instruction, team consultation, etc. + Develops and directs the implementation of an annual program evaluation plan and performance improvement activities, including CQI projects. + Develop and implement plan of care based on assessment of patient’s/family learning needs. + Ensures that DSME/T program accreditation requirements are met and maintained. + Maintain 15 hours of continuing education annually as it relates to the profession. + Performs other duties and responsibilities as assigned.
