Registered Nurse (RN) - Ambulatory Anticoagulation Program Coordinator (Remote)

Open due to growth - Part time position - 20 hours a week

Previous anticoagulation experience within the last 3 years in a ambulatory care setting is highly preferred!

Major Responsibilities

Standardization and sustainment of nursing practice workflow including documentation Policy & guideline maintenance Evaluate evidence-based models of care resulting in safe, efficient and quality care Creation of training programs and onboarding tools Primary consultant for anticoagulation related questions Consult with operations leaders to create new structured care clinics Collaborate with PG Anticoag Specialty Council regarding information sharing (Q&A, annual conference, Just in Time learnings, etc.) Establish program metrics and audit tools Collaborate on developing necessary action plans with AAC nursing & local operations leaders Co-facilitate PG Anticoag Specialty Council and weekly safety huddle Analyze PSERs (Origami) and safety huddle reporting to identify trends and make recommendations for improvement opportunities Attend/represent at CI monthly anticoagulation meetings Daily follow up on default BPA inbox 

Licensure, Registration, and/or Certification Required:

Valid Registered Nursing license in either IL or WI 

Education Required:

Bachelor’s Degree in Nursing 

Experience Required:

Typically requires a minimum of 3 years of experience in anticoagulation management 

Knowledge, Skills & Abilities Required:

Knowledge of clinical and operational elements in the Medical Group  Knowledge of Epic electronic health record (EHR)  Knowledge of change management principles  Skilled communicator – verbal, written, presentations  Skilled project manager Skilled data analyst  Skilled user of Microsoft Office Suite Ability to negotiate and solve problems Ability to influence work and decisions without direct reporting relationships  Ability to effectively convene and lead a team including creating a vision, building commitment, establishing and meeting timelines, and positively impacting patient outcomes Ability to work effectively with multiple people in cross-functional teams across the care continuum Ability to work independently and in ambiguous environments
