This role performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures. This Nursing schedule includes; + 48 hours every two weeks + Day/eveing shift; 12 hour shift + Every third weekend rotation + Every other holiday rotation **Meadows on Fairview TCU** is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch.  We offer a home like atmosphere where you feel like you belong the minute you arrive!  Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow. **Job Responsibilities:** + Monitors personal cares provided to assure consistency with each individual plan of care + Administers medications and performs treatments per physician orders and facility policies + Provides data relevant to the development of the plan of care + RN – Assesses plan of care, resident goals and revises, modify plan of care as necessary + Provides reports and documentation related to incidents, change of status and cares provided **Required:** + Vocational/Technical Training or Associate Degree - Registered Nurse + Current MN RN license + AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers + BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification.
