Job Description As the Program for Assertive Community Treatment (PACT) Program Nurse, you are a member of a multidisciplinary team, utilizing your nursing skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to (PACT) clients under the clinical supervision of the team leader and the prescriber. A team member equally shares the responsibility to perform various activities and interventions that support (PACT) clients in the community. DUTIES AND RESPONSIBILITIES: Integrates the Prism Model for effective treatment of serious and persistent mental illness into everyday practice and intervention. Provides person-centered, strength-based, trauma-informed care Ensures that clients receive effective, understandable, and respectful care that is provided in a manner compatible with client's cultural beliefs and practices and preferred language Carries out medical functions, including essential health and medical assessment and education; coordination of health care providing clients in the community; psychiatric medical assessment, treatment, and education; and psychotropic medication administration Acts as a lead nurse, when designated by the team leader, provides nursing supervision to the team. As assigned, designated as the assigned 'medication nurse' for the day. Being available to administer medication Updating medication orders and handling tasks related to medication distribution Assessing and triaging, in consultation with the team prescriber, any emergent medical problems or needs Provides medication administration and medical services: Under the direction of the team prescriber, and in collaboration with other registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures, including transcribing, administering, evaluating, and recording medications prescribed by the team prescriber; assess and document medication effectiveness, complications, and side effects; and arrange for required labs according to protocol. Organizes and manages the system of getting medications to clients, administering and tracking the medication supply, and ordering and offering education to clients to gain knowledge and independence. Maintains the PACT headquarters medical supplies Preventative medical care: tracks, educates, links assessed medical/dental needs with a community provider, collaborates with providers, accompanying to appointments, providing support and advocacy. Along with the team, on an ongoing basis, reviews the required outcomes to evaluate community tenure and independence. Works with the team to complete the Tool for Measurement of Assertive Community Treatment (TMACT) Provides service coordination for an assigned group of clients, including coordination and monitoring the activities of the individual treatment team (ITT); assumes primary responsibility for developing, writing, implementing, evaluating, and revising the overall Community Service Plan in collaboration with the client and ITT; provides therapy utilizing evidence-based practices (i.e., CBT-P, CBT, DBT, CAMS, MI), symptom management, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences. Works with the team to complete a Comprehensive Assessment as an engagement tool and to understand and respect the client's views of the ways mental illness impacts their life and how they want to be supported in their process of recovery Utilizes evidence-based assessment tools that support matching a client's needs to their services In partnership with the client and ITT, develops the Community Service Plan within seven days of the team finalizing the comprehensive assessment. Provides de-escalation techniques to aid the client(s) in managing a crisis, providing intervention to decrease risk, manage symptoms, and maintain safety Maintains the electronic card-ex Performs shift management in coordination with other ACT shifts managers Participates in the Daily Organizational Meeting Provides on-call crisis intervention covering nighttime and weekend hours Documents client progress, activities, and outreaches in the Electronic Health Record Practices a Housing First approach Practices engagement skills across the adult lifespan Support:?provides practical help and support, advocacy, coordination, problem-solving, training, and supervision to help clients obtain the necessities of daily living; assist clients in finding and maintaining safe housing, coaches clients to learn and carry out ADLs, assist and support clients to perform household activities, including house cleaning and laundry, teach money-management skills, nutrition education, meal planning, grocery shopping, access transportation, help clients to have and efficiently use primary care, dentist, and other medical specialist, support clients with structuring their time (community resources, social and leisure activities, hobbies, etc.) Ongoing assessment, symptom education, direct clinical services Awareness of and offers a range of valuable community activities and linkages that support the client's Community Service Plan. Performs other duties as assigned QUALIFICATIONS, EDUCATION AND EXPERIENCE: Pre-Employment Education/Experience Associate's degree in nursing, bachelor's degree, preferred 3+ years in leadership, management role, experience in behavioral healthcare, and working with individuals with mental health conditions preferred Licensed in the State of Massachusetts as a Registered Nurse Preference will be given to candidates that meet the cultural and linguistic needs of the geographic area Relevant Post-job Offer Testing/Conditions Knowledge and practice of FHR, DPH, and DMH policies, regulations, and standards Must complete PACT Module Training within 30 days of hire Participate in other FHR required/ recommended training according to the training calendar Maintain required clinical license Meet or exceed professional development goals Demonstrates fidelity to the PACT model Ability to make contacts and establish relationships with internal and external supports that allow for the program to function within the more extensive system?
