Registered Nurse - Per Diem

Our Registered Nurse - Per Diem plays a vital role by caring directly for our participants and covering any center-based nursing roles on a per diem basis within a designated region. The Registered Nurse - Per Diem is accountable for providing nursing care to participants as assigned, as well as supporting with clinic overflow and day center triage, checking vital signs, performing plan of care (POC) testing, administering medications, and assisting with wound care and any other clinical needs in accordance with the RN scope of practice.

Essential Job Duties:

Own the outcomes and quality of care for assigned participants, working in tandem with the PCP and utilizing clinic resources to function at the top of their license Conduct PACE required nursing assessments (minimally every 6 months), educate participants and their support networks on acute, chronic, and end-stage conditions, perform both urgent and routine virtual or in-person follow-up visits as determined by each participant’s risk stratification and needs as a direct extension of the PCP Develop and implement a discipline-specific POC in collaboration with the interdisciplinary team (IDT), participants, and families to ensure delivery of high-quality, holistic, and compassionate care, as well as capture assessments, findings, relevant information, and respective actions through timely and compliant documents in EHR Coordinate care for selected participants with highest level of complexity, including those at the hospital, ER, and skilled facility level of care to ensure seamless transitions of care and participant satisfaction, and to prevent unfavorable outcomes, inappropriate utilization of resources, and unnecessary hospitalizations Record patient care delivered as planned to include any variation, and implement nursing measures related to impending or associated problems Identify emergency situations and independently initiate therapies within scope of practice as needed Attend as an active member of the IDT, ensuring the nursing needs of participants are assessed, planned, and addressed

Job Requirements:

Graduate from an Accredited School of Nursing with an unencumbered RN license Minimum of two (2) years of nursing experience in a clinical setting with a frail or elderly population Comfortability working a per diem work schedule that aligns with business needs Ability to travel as needed to ensure daily coverage within assigned region Excellent clinical, organizational, and communication skills in a clinical setting Experience multitasking to contribute to projects, respective activities, timelines, and process improvement initiatives
