Summary:
Under the general supervision of the Director Community Health Institute provides nursing services consistent with community outreach education and screening programs.  

Responsibilities:

Provides health education programs consistent with ability education and training.
Provides required nursing support for health screenings.
Assists with follow up for all screening participants with abnormal test results and/or no primary care provider.  Assures contact with such individuals until appropriate care provided.  
Represents the department at community events to provide outreach education and screening services.
Acts as resource to staff in the referral service for callers experiencing barriers to accessing care.  
Improves quality of results by studying evaluating and recommending changes in processes; and implementing approved changes.
All essential functions performed in this position respect the age gender language and other cultural and social needs of the patients.
Maintains quality assurance safety environmental and infection control in accordance with established policies procedures and objectives.
Performs other duties as assigned.

Other information:

Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.

BASIC KNOWLEDGE:
Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes human development stages and cultural patterns in each step of the care process. 
Graduate of an accredited school of nursing with current license to practice as a Registered Nurse in the State of Rhode Island.
Must have current American Heart Association Basic Life-Support for Healthcare Providers certification. 
Bilingual preferred 
EXPERIENCE:
Three years� related and progressively responsible clinical and community experience and basic computer skills.  Teaching and community health experience preferred.  
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Prolonged standing and walking.  May be required to lift push or pull equipment up to 35 pounds.   Repeated bending squatting stooping.
