We are seeking a dedicated and experienced Registered Nurse to join our team as a Patient Care Manager in Home Health. This role is pivotal in ensuring high-quality care and support for our patients. Key Responsibilities:

Oversee and manage patient care services in a home health setting. Coordinate with multidisciplinary teams to ensure comprehensive patient care. Develop and implement individualized patient care plans. Monitor patient progress and adjust care plans as necessary. Ensure compliance with healthcare regulations and standards. Provide education and support to patients and their families. Supervise and mentor nursing staff, promoting professional development.

Qualifications:

Current Registered Nurse (RN) license. 1 year experience as a Registered Nurse. Minimum of 2 years of experience in home health or related field. Proven leadership and management experience. Excellent communication and interpersonal skills. Strong critical thinking and problem-solving abilities. Ability to work independently and effectively manage time.
