Perform initial nursing assessments and ongoing cognition screens at start of care Develop an individualized care plan for each new client including recommendations for care hours Perform follow-up visits as per state licensing requirements and at client status changes Accurately monitor and record data regarding patients' conditions Collaborate with community care partners to support the provision of quality care to each client. Supervise and support CNAs to ensure that they are fully equipped to provide the highest quality care to our clients Other duties as necessary to the support the Lead Field Nurse.Compensation: $35.00 - $40.00/Hour & Pay Per Assessment
RN Qualifications:
Active RN Nursing license Maryland required. 1+ years working as a nurse in a home care, home health or hospice setting (preferred). Evidence of continued learning to maintain proficiency of professional and technical emerging knowledge. Experience working with an older adult population preferred. Exceptional problem solving, prioritizing, and multitasking skills. Strong interpersonal skills that allow the development of rapport between the clients, caregivers, and internal staff. Possess initiative to make independent decisions when required. Ability to act swiftly in emergency situations.License/Certification:
