Registered Nurse – Palliative Care 
Location: Outpatient | Serving Carroll County, Baltimore County, and Baltimore City 
Organization: BridgingLife Hospice, part of LifeBridge Health 

 

About the Role: 
BridgingLife Hospice is seeking a compassionate, experienced Registered Nurse (RN) to join our Palliative Care team. This full-time position provides vital outpatient support to patients in Carroll County, Baltimore County, and Baltimore City. The RN will play a key role in coordinating and delivering comprehensive palliative care services that focus on comfort, dignity, and quality of life. 

 

Key Responsibilities: 

Deliver and document medically necessary nursing services in accordance with provider orders. 

Assess patient and family/caregiver needs; coordinate appropriate services (e.g., DME, home health care, hospice). 

Collaborate with the palliative care provider to develop individualized plans of care aligned with treatment goals. 

Provide disease management education and guidance to patients and families. 

Conduct comprehensive nursing assessments, addressing emotional, physical, spiritual, and social needs. 

Collaborate with the interdisciplinary team to ensure integrated, patient-centered care. 

Support the development and application of clinical practice standards to ensure high-quality care. 

Maintain compliance with all federal, state, and local regulations, as well as BridgingLife and LifeBridge Health policies. 

Perform other related duties as assigned. 

 

Qualifications: 

Active Registered Nurse (RN) license in Maryland (MBON required) 

Current AHA Basic Life Support (BLS) certification required 

Must meet Fleet Safety requirements 

Prior experience in palliative care or hospice is preferred 

Strong interpersonal, assessment, and organizational skills
