Registered Nurse/ Licensed Practical Nurse
**Responsibilities** + Monitor, assess and report changes in medical conditions for individuals with developmental disabilities + Train staff to provide high-quality care through ongoing education, trainings and competency evaluations + Coordinate services and supports, prioritizing health and safety + Participate in team meetings, planning and implementation of person centered health care + Demonstrate knowledge of nursing standards of care + Comply with HIPAA regulations + Ensure compliance with Centers for Medicare and Medicaid Services (CMS) guidelines + Communicate with multidisciplinary team members to meet individual patient care needs + Possess strong written and verbal communication skills + Applicants must be flexible, able to work in a team and self-directed environment, and possess excellent time management skills RN/LPN will assure the needs of the people supported are met by coordinating services and advocating for their best possible health.
