Registered Nurse Intake liaison
Position OverviewUnited Health Services (UHS) is seeking a Registered Nurse Intake Liaison. This role involves coordinating patient intake processes to ensure efficient and effective care delivery. The Registered Nurse Intake Liaison will work closely with healthcare providers, patients, and families to gather necessary medical information, facilitate communication, and support a smooth transition into appropriate services. The ideal candidate will have strong clinical knowledge, excellent communication skills, and the ability to manage multiple tasks in a fast-paced environment. Primary Department, Division, or Unit: Nursing Support Services, UHS Home Care Primary Work Shift: Day Regular Scheduled Weekly Hours: 24 Compensation Range: $31.15 - $46.72 per hour, depending on experience ----- Job Responsibilities Essential Functions: + Coordinate patient care with referral sources and analyze appropriateness of admissions. + Act as a liaison between patients and physicians. + Ensure safe, effective nursing care using clinical expertise and problem-solving. + Accept referred patients who meet admission criteria and triage based on clinical status. + Visit hospitals and skilled nursing facilities to coordinate referrals. + Participate in patient conferences to coordinate services for complex cases. + Obtain start-of-care or treatment orders from physicians. + Record and forward required patient information to the Intake Coordinator. + Educate patients, physicians, and healthcare staff about services. + Maintain professional contacts to promote and coordinate care services. + Provide follow-up reports to discharge planners and facility personnel. + Uphold compliance with organizational policies, HIPAA regulations, and participate in continuing education. + Attend pertinent agency meetings.
