Work where you matter.** **A Brief Overview** Provides individual and group diabetes self-management education, conducts community and health professional education related to diabetes, and serves as a resource for the nursing staff relative to diabetes education and basic nursing care of the diabetes patient. **What you will do** + Plans, conducts and evaluates system diabetes disease management processes and systems. + Serves as a clinical resource person for providers and nursing staff related to the care and education of diabetes patients. + Supports clinics in disease management and access to diabetes education. + Identifies opportunities for program development and participates in special projects as assigned. + Develops, conducts, and evaluates training program for clinic staff to provide diabetic education. + Collaborates with diabetes management and education resources across the system. + Maintains expert knowledge in diabetes education and case management tactics. **Essential Qualifications** The individual must be able to work the hours specified.
