**SUMMARY OF POSITION:**
Responsible for providing guidance and coordination of care for patients and their families through the process of diagnosis and treatment. Responsibilities also include daily operations of program and providing direct patient care. Also responsible for the coordination of marketing and program promotion.
ESSENTIAL DUTIES- The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
+ Responsible for the coordination of care for patients, including providing direct patient care as required.
+ Act as Liaison between physician and patient.
+ Prepare casework as necessary.
+ Coordinate weekly Program staff meeting of combined department members including speakers and agenda.
+ Coordinate patient education and programs including marketing and community outreach. Maintain relationship with extramural organizations and manufacturers regarding the latest advancements in standards of practice, policies and treatment drugs and devices.
+ Provide leadership to personnel and assist with management and supervisory duties, including budget, quality performance indicators and patient satisfaction results.
+ Lead nursing Team to provide continuity of patient care.
+ Manage and monitor financial resources for project development.
+ Arrange home care for post op patients.
+ Facilitate the Support Group.
+ Actively participate in hospital committee groups.
**MINIMUM QUALIFICATION(S):**
+ Three (3) years of nursing experience
+ Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
