St. Vincent’s is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network and is the system’s second-largest hospital. *Job Summary* Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments (HODs) and Medical Group, in a variety of clinical settings. This position facilitates care for a varied population of patients, which includes nursing standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family. *Responsibilities* · Provides nursing care nursing care within a non-acute setting in collaboration with the inter-disciplinary team and provider according to the patients physical, emotional, spiritual, cultural, and educational needs of the patient and family. · Coordinates and provides holistic patient centered care. · Utilizes evidence-based practice in accordance with the department and/or medical group policies and procedures to ensure safe quality care and customer service. · Incorporates art, advocacy, science and ethics in nursing effectively in their daily practice. · Demonstrates and actively supports research utilization through inquiring into evidence-based practices. · Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family. · Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients. · Maintains confidentiality of all patient information · Assumes responsibility for self-improvement in collaboration with the leadership team by identifying, communicating and seeking resources or instruction/validation to meet own learning needs, and is self-directed in maintaining currency of validations. · Embodies the HHC leadership behaviors and company values and effectively utilizes the HHC operating model. · Completes required learning by designated deadline. · Maintains a clean, neat, and safe environment for patients and staff, ensuring supplies and equipment are stored in an organized and efficient manner. · Performs other related duties as required. *Minimum Requirements* · Graduate of an accredited School of Nursing. · Associate’s degree in nursing. Bachelors preferred. · Successful completion of nurse residency, if applicable. *Licensure, Certification, Registration* · Registered Nurse, State of CT. · Certified BLS Provider credentialed from the American Heart Association (AHA). · Additional certifications may be required in specific specialties. · Certification in a specialty a plus. *Language Skills* · Must be able to effectively communicate information in English. · Ability to speak another language in addition to English a plus. *Knowledge, Skills and Ability Requirements * · Applies the principles and practices of nursing. · Membership in a professional nursing organization is preferred. · Demonstrated ability to develop, deliver and evaluate patient care. · Knowledge of high reliability. · Strong communication skills, both verbal and written. Use communication skills to foster a healthy work environment and facilitate positive patient outcomes and experience. · Demonstrates cultural sensitivity in all interactions with patients/families and co-workers. · Strong knowledge of care for age specific populations. · Strong ability to work as part of a collaborative team. · Ability to be flexible with changes in schedule and locations. · Self-directed in clinical responsibilities. · Ability to manage and organize multiple clinical priorities. · Strong computer skills and use of industry applications/programs. Ability to learn new technology and programs as advancements and changes are implemented. ** · Ability to adhere to OSHA regulations and Infection Control Standards. · Ability to adhere to HIPAA requirements.
