What You’ll Do:• 🩺 Direct Care & Case Management:Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.• 🏥 Health & Safety Oversight:Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individuals’ health and medical needs.• 🤝 Collaborate with Treatment Team:Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.• ⚠️ Emergency Response:Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.• 📝 Documentation & Care Plans:Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.• 📅 Appointment Scheduling:Ensure timely scheduling of medical appointments and services for individuals in your caseload.• 👩‍⚕️ Staff Training:Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.• 💊 Medication Management:Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.• 🏥 Hospitalization & Discharge Coordination:Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.• 📑 Health Record Monitoring:Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.• 💼 After-Hours On-Call:Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.• 🛡️ Safety Compliance:Follow safety guidelines to ensure a safe working environment and report any hazards.• 🌍 Program Coverage:May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals. What We’re Looking For:• ✅ Education:Bachelor’s or Master’s degree in Nursing, with at least one year of professional experience in delivering health care to individuals with Intellectual/Developmental Disabilities.• ✅ Licensing:NYS RN Certification is mandatory.• ✅ Driver’s License:Valid unrestricted NYS Driver’s License and reliable transportation for travel between locations.• ✅ Skills:Ability to triage urgent and crisis situations, problem-solve, and maintain focus in challenging situations.• ✅ Flexibility:Must be flexible with days, hours, and location based on agency needs.• ✅ Vaccination:Must be fully vaccinated (COVID-19).
