We are seeking a compassionate and experienced Registered Nurse to deliver high-quality home health care services. This role involves conducting clinical assessments, administering treatments such as wound care and medication, and executing physician orders. The RN will collaborate with home health aides and multidisciplinary teams to ensure coordinated, patient-centered care.

ResponsibilitiesConduct clinical assessments and administer treatments such as wound care and medication.Execute physician orders accurately and efficiently.Collaborate with home health aides and multidisciplinary teams to ensure coordinated, patient-centered care.Utilize HomeCare HomeBase and OASIS documentation systems effectively.Manage a caseload independently and prioritize patient visits efficiently.Educate patients and families about care plans, medications, and recovery strategies.Provide respectful and effective care across diverse populations.Respond calmly and effectively to urgent changes in patient condition or environment.Essential SkillsActive Connecticut RN License.Minimum of 2 years of nursing experience, with at least 1 year in home health.Proficiency in HomeCare HomeBase and OASIS documentation.Strong communication, organizational, and critical thinking skills.Reliable transportation for patient visits.Additional Skills & QualificationsBLS certification (preferred).BSN degree (preferred but not required).Experience with chronic disease management.Certification in wound care or IV therapy.Proficiency with mobile documentation tools, EMR systems, and telehealth platforms.Knowledge of HIPAA compliance and secure documentation practices.Work Environment

The role operates from 8:30 AM to 5 PM, with travel required within the surrounding towns of Greenwich in Fairfield County.

Pay and Benefits

The pay range for this position is $95000.00 - $115000.00/yr.

Paid Time Off & Holidays
15 PTO days annually after 90 days, usable for vacation or sick time.
Accrual: +1 day per year of service, up to 20 days max.
8 paid holidays: New Year’s Day, MLK Day, Memorial Day, Juneteenth, July 4th, Labor Day, Thanksgiving, Christmas.
Flexibility: Unused PTO beyond 15 days can be paid out upon request.
