Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. Our Community - Generations Family Health Center: https://genhealth.org/our-community/ Location:Putnam, CT Registered Nurse Role: + Assess and identify patient needs, providing safe, evidence-based nursing interventions that promote physical and mental wellness. + Deliver educational visits focused on chronic disease management (both mental and physical health), empowering patients to better understand and manage their conditions. + Administer medications, including long-acting injectable medications for behavioral health treatment, in accordance with established protocols. + Conduct telephone triage and support crisis intervention needs in collaboration with the behavioral health and medical teams. + Provide follow-up care and outreach for patients discharged from the hospital or emergency department following a behavioral health-related event. + Participate in interdisciplinary team meetings, contributing to the development of holistic, individualized care plans. + Work alongside a supportive team of nurses, primary care clinicians, and behavioral health clinicians to ensure seamless coordination of care.
