This includes facilitating appropriate care, assisting patients and families in navigating treatment, and ensuring efficient progression through the healthcare continuum.
What You Will Do:Complete comprehensive health assessments, including physical, psychological, social, environmental, financial, and functional statusDevelop treatment goals and care plans in collaboration with providersServe as a clinical expert and resource for staff and patientsParticipate in performance improvement initiativesMonitor and adjust care plans through interdisciplinary collaborationApply critical thinking and problem-solving to support individualized patient careWhat You Will Need:Graduation from an accredited nursing programAssociate Degree in Nursing (ADN) required; BSN preferredGrand Rapids candidates with ADN must enroll in a BSN program within 6 months of hire and complete within 4 yearsCurrent RN license in the State of MichiganChemo/Biotherapy Certification (or must obtain within 6 months of hire)Minimum 1 year of RN experience requiredOur Commitment
