Description:Mercy LIFE, a member of Trinity Health PACE, is seeking a Licensed Practical Nurse for its location in W. Springfield, MA. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

Per-diem position

We are seeking a compassionate Registered Nurse to join our Trinity Health PACE team at our Mercy Life of MA. We have an opening in both our Clinic!

Trinity Health PACE ministers exclusively to the elderly through a coordinated PACE care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.  Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center.

What you will do:

The Registered Nurse (RN) is responsible for providing oversight and management to the provision of service delivery to participants. Provide skilled nursing care, prescribe treatments, and develop appropriate plans of care to participants according to professional nursing standards and primary care orders.

What you will need:

Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the state of PACE Organization. BSN preferred. Active CPR Certification required.Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.Valid driver's license in good standing and proof of insurance required.Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services. Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.Physical mobility that includes movement from place to place.Must be able to sit for long periods of time.Physical agility, that includes ability to maneuver body while in place.Dexterity of hands and fingers.Coordination, including eye-hand, hand-foot.
