This position is responsible for the day-to-day management of the cardiac rehab department. This role will guide patients through a 12 week exercise program, aimed to reduce cardiac events, improve heart failure outcomes and improve quality of life.  The Registered Nurse coordinates with members of the health care team to educate patients on severity of condition and coordinate complex care needs. The RN acts as an educator, resource, and advocates for members and their families to ensure a maximum level of independence.  This role works in conjunction and collaborates with a multidisciplinary care team, which includes providers, nurses, case managers, social workers, and educators. 

Primary Responsibilities:

Functions independently, utilizing the nursing process to triage member biometric and subjective data and member telephone calls in accordance with approved protocols ensuring safe, quality patient care in the least restrictive care setting Completes patient assessments and relays concerns to appropriate care providers when needed, utilizing sound professional judgment and clinical decision making skills Works closely with specialty and PCP providers and clinic staff to identify members with heart failure related exacerbations and coordinates member visits with specialists / PCP as needed Assesses member or caregiver to determine their understanding of the severity of their disease process and ability to carry out instructions provided by RN or clinic staff Establishes a trusting relationship with members, caregivers, clinic staff and providers through regular communication and follow up regarding patient needs and concerns Utilizes appropriate motivational interviewing techniques necessary for providing effective education and encouraging adherence with recommended healthcare regimen Makes referrals to outside sources Researches specific clinical situations and collaborates with multidisciplinary care team including physicians, nurses, case managers, social workers and other educators/health coaches to assure that a well-coordinated treatment plan is formed Engage patient, family, and caregiver regularly to assure that a well-coordinated treatment plan is put into action and any new needs or concerns are identified in a timely manner Manages patient case load in an efficient and effective manner utilizing time management skills to facilitate the total work process Provides constructive information to minimize problems and increase customer satisfaction Seeks ways to improve job efficiency and makes appropriate suggestions following the appropriate chain of command Enters timely and accurate data into electronic medical record and other applications necessary to communicate patient needs and ensure complete documentation of patient visits and phone calls Performs all other related duties as assigned

Required Qualifications:

Education:
Bachelor’s degree in Nursing, or Associate’s degree in Nursing combined with five or more years of experience Current, unrestricted RN license to practice in the applicable state of employment 2+ years of diverse clinical experience caring for acutely ill patients with multiple disease conditions 1+ years of cardiac, medical-surgical and/or critical care experience as a Registered Nurse Experience related to patient education and/or motivational interviewing skills Proficient in PC software computer skills such as Microsoft Word, Excel, Power Point, and other departmental specific programs/applications Proven excellent verbal and written skills Proven planning, organizing, conflict resolution, negotiating and interpersonal skills Proven ability to interact productively with individuals and with multidisciplinary teams Proven independent problem identification/resolution and decision making skills Proven ability to prioritize, plan, and handle multiple tasks, demands simultaneously Ability to travel occasionally

Preferred Qualifications:

CCU/ICU experience Cardiology experience BSN degree with current, unrestricted RN license to practice in the applicable state of employment Experience working with geriatric patient population Bilingual (Spanish) language proficiency

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
