ESSENTIAL DUTIES AND RESPONSIBILITIES The Registered Nurse will serve young adults and families with children headed by young adults ages 18 to 25 years old providing health care and management coordination. The Registered Nurse will: + Conduct assessment of the health needs of each resident including identifying barriers to access health services and health related resources in the community + Create a health and wellness action plan tailored to each client’s needs, working collaboratively with healthcare providers and other community-based organizations to ensure client receives comprehensive and coordinated care and an increase access to healthcare services + Assist tenants in managing medications and navigating appointments + Promote health, prevention, and wellness informational activities for residents specific to their determined health needs + Provide access to educational resources in order to increase client awareness of social determinants of health outcomes as well as how to manage chronic conditions + Provide clients with ongoing follow-up, conduct motivational interviews, and assist with health goal setting + Create a network of community medical and mental health providers and resources; provide information and referrals to clients as needed + Assist clients with medication review and understanding directives from Primary Care Physicians (PCPs) in order to address chronic health conditions and decrease utilization of hospitals and other high-cost interventions + Provide consistent communication to clients via Friendly Visits (FV) and Telephone Reassurance phone calls (TR) + Assist residents in accessing necessary medication as well as any medical equipment that will ensure resident safety and housing stability + Together with the Supervisor, organize curriculum for health and wellness educational workshops for residents + Facilitate health-focused workshops and presentations based on resident needs and feedback + Track and report on the outcome of referrals and the impact of services on resident health and housing stability + Assist with acute care coordination + Other related duties based on the needs of the program COMPETENCIES + Dynamic, able to multi-task, problem solve, and work independently + Team player and willing to work with a diverse population + Trained in child development, mandated reporting, and family dynamics + Trained in harm reduction and housing first services approaches + Experience with evidence-based practices and/or evidence-informed services including, but not limited to: + Positive Youth Development + Recovery-Oriented and Trauma-Informed Case Management + Critical Time Intervention + Ability to problem solve and work independently + Must be able to travel and work evening and weekend hours + Commitment to RiseBoro’s mission of unleashing the potential of communities to thrive, no matter the odds EDUCATION AND EXPERIENCE + Licensed Registered Nurse + At least 2-3 years of relevant experience working with special needs populations Qualifications It is the policy of RiseBoro to provide equal employment opportunity to all employees and applicants for employment.
